In bad taste.

I am about to reveal a heretical belief for a porn store clerk; I think ass-to-mouth scenes are ill done. I find the concept exceedingly icky.

Those that enjoy that little facet in their movies are more than welcome to it. It hasn't hindered my abilities as a salesman before, it won't in the future. It's not like someone looking for a movie featuring someone skull-fucking a dead puppy. (That movie would be violating a few laws depending on what state one is in.)

However, I don't have a high opinion of A2M and that tends to bleed across to those who enjoy it. My biggest reason is that it's not sanitary. The rectum is an organ for the elimination of solid metabolic waste. Thus, regardless of what recreational purpose it may find itself conducting on occasion, it's the last stop before embarkation to the sewers. Fecal material is often cited as a major vector for disease. E. coli, salmonella, a variety of endoparasites are all easily transmitted fecally. Coprophagia falls outside normal human behavior. It's physically unhealthy.

In addition, most people know that if you're going to be playing with both a vagina and rectum, you can't just mix and match at whim. You may introduce a toy or phallus that has been utilized vaginally to the rectum without much worry, but you really don't want to go the other direction. Ignoring this risks introducing nasty little critters that can cause a seriously unpleasant infection. The mouth may be a bit less susceptible to infection than a vagina, but most of the things you would rather not contract are well adapted to pass the oral cavity.

There is no way for me to believe that in the process of fucking someone in the ass, they aren't going to have anything transfer from the rectal walls to the penis, I don't care how much lube is involved. The more energetic they go about it, the higher the degree of transfer. And if that cock is going to pass someone's lips after, they're going to be ingesting it all.

So what about people that like scat play? What about them? For the most part, as I understand it, they aren't ingesting. People that like water-sports? Urine's pretty clean for an eliminated waste product. It even has some anti-fungal properties. Beyond that, these activities are mostly negotiated and both sides know the risks ahead of time. My impression of A2M is that it's bit less informed. In the event it has been negotiated prior, any reservations regarding the participants are right out.

I suppose someone could have an enema prior to make sure everything is all spot-less and clean, but that takes planning and most of the seeming appeal is that it's spontaneous. Besides, getting enemas on a frequent basis risks some physical consequences that aren't exactly fun.

Anyway, that's my own take on the whole thing.
